Terms and Condition for Assessments

Your Information and Privacy

To provide you with a quality assessment, we need to collect and record information about you. We take great care in keeping your information secure. There are some instances where we may share your information:

· Where sharing the information may prevent you or someone else from serious harm

· Where a court orders the information to be provided

· Where your psychologist is seeing supervision to ensure you are receiving the best quality assessment

In accordance with relevant legislation, your information will be stored securely for 5 years after you discontinue services with us and it will then be distroyed.

Assessment Process

Multiple options are available for assessments, and we will help you determine the best fit for you, depending on factors such as your goals and the cost. We will confirm the elements and pricing of your assessment with you before it begins. If it seems that adjustments to the assessment may be necessary, you will be informed, and the earliest opportunity and your approval will be obtained before making these adjustments. Verbal and written feedback will be provided after the assessment. Up to 3 weeks is required to prepare this feedback after the final assessment session.

Payments

Fees vary depending on the nature of your assessment. The fee for your assessment and a payment plan will be provided prior to starting the assessment. Payment of all fees is required before the feedback session.
